Mental Health Targeted Case Manager

Douglas Gardens CMHC is a non-profit organization that has been providing comprehensive mental health services to the local community for over 40 years.

Douglas Gardens CMHC is hiring full-time Case Managers for the North Miami outpatient office. Case Managers assist clients in meeting mental health and social service needs by advocating, linking, monitoring, and planning for services.

What does a Case Manager do?

• Conduct case management assessments.
• Develop treatment plans and case management service plans.
• Monitor services and progress toward goals, leading to reassessment and development of new service plans or linkages.
• Intercede on behalf of client to ensure access to needed services and/or resources.
• Link clients to local community resources.
• Complete prior authorizations for services or medications.
• Meet with clients in off-site locations including client’s home for bi-monthly home visits.

Requirements for Case Managers:

• Minimum requirement of a bachelor’s degree in the field of human services - Social Work, Psychology, Criminal Justice or a related discipline from an accredited College or University.
• Minimum one-year of experience in a case management like position, working with a similar client population, preferred.
• Bilingual English/Spanish or English/Creole.
• Successfully complete background screening and drug screening.
• Individual must have a valid Florida Driver’s License with clean driving history and automobile liability coverage as required by Florida law.
• Completion of AHCA approved mental health targeted case management training within three months of hire – Provided by the company.
• Attain Florida Certification Board’s Certified Behavioral Health Case Manager Certification within six months of hire –application expense reimbursed by employer.

This is a full-time position, Monday-Friday 8:30am-5:00pm.
