POSITION DESCRIPTION
Title: Mental Health Court Case Manager
Service Area: Boone, Kenton, and Campbell Counties in Kentucky
Reports to: Administrator of the Northern Kentucky Regional Mental Health Court
Position Type: Full time 12-month contract
Position Overview:
The Mental Health Court Case Manger serves as multi-function member of the Northern Kentucky Regional Mental Health Court (NKRMHC) Services Team and is responsible for oversight, coordination, delivery of services and supports to clients of the NKRMHC. This team member works collaboratively with all other NKRMHC staff and other community agencies/professionals to ensure effective treatment planning, service linkage, treatment compliance and coordinated service delivery to clients of the NKRMHC across the three-county service region of the court.
Minimum Qualifications:
Bachelor’s degree in social work, or related human service field, or Bachelor’a degree in criminal justice or related field. Excellent written and verbal communication skills. Proven ability to meet or exceed work productivity targets and program and client outcomes. Reliable transportation and automobile insurance, and clean driving record required.
Responsibilities:
- Refer and assist with linkage for clients to needed services
- Conduct frequent and regular community-based meetings with clients including home visits
- Assess and develop individual service plans for each client
- Monitor client symptoms, behavior, relapse, and medication compliance
- Assist clients with transportation to medical, psychiatric, and other appointments as indicated
- Transport clients as needed
- Develop regular progress reports for mental health court
- Attend court hearings as required
- Participate in regular team meetings with NKRMHC staff, community treatment teams, and probation and parole officers
- Meet individually with clients to review treatment plans, progress and to update goals
- Work as an integrated member of the NKRMHC team to understand and assist other agencies as needed
- Assist with crisis intervention and management and seek emergency psychiatric services for clients as needed
- Accurately complete required documentation in timely manner
- Meet or exceed required individual productivity standards
- Coordinate and manage case records
- Effectively contribute to the accomplishment of targeted outcomes for the program
- Flexible schedule including weekends and evenings as assigned
- On-call crisis line monitoring as assigned
- Other duties as assigned
