Communications and Learning Planner - Planner Principal State

Your work will empower clinics and community partners to deliver better-coordinated, equitable, and high-quality healthcare.
As a Communications and Learning Planner, you will be at the forefront of planning, developing, and implementing communication strategies that promote the HCH program. Additionally, you’ll support learning activities designed to build capacity and support practice transformation among certified clinics, ensuring improved patient care coordination across primary care teams, specialists, and community partners.
Primary responsibilities include:
- Lead statewide communication strategies to promote HCH programs and practice transformation goals.
- Develop and disseminate tools, resources, and presentations to support clinics and stakeholders.
- Manage grants, contracts, and vendor relationships to advance program initiatives.
- Facilitate workgroups and advisory committees to support program sustainability efforts.
- Support strategic planning, evaluation, and professional development activities for HCH.
This is a hybrid telework position that requires quarterly in-person meetings in the downtown St. Paul location as well as occasional statewide trainings and conferences. Telework will be based on the business needs of the unit and is at supervisory discretion.
Minimum Qualifications

- Three (3) years of experience developing or managing projects. Experience must include 4/6 of the following:
- Creating oral communication materials.
- Creating written publications.
- Contract or vendor management.
- Policy or process development.
- Research of emerging best practices of subject matter.
- Creating reports or presentations on findings.
- Experience with Microsoft Office software, including Adobe Pro.
*A degree in a related field such as Communications, Public Administration, Business Administration, Information Technology, Public Policy, Health Informatics, Public Health, Health Care Management, Education, Community Health, Data Science, or Marketing may count towards professional experience as follows:
- Associates = 6 months
- Bachelor’s = 12 months
- Master’s = 18 months
- PhD = 24 months
Qualifications to be further assessed during the interview process:
- Knowledge of primary care and/or healthcare systems.
- Knowledge of patient-centered care, health equity, and quality improvement principles.
Preferred Qualifications
- Adobe Captivate 2019 experience
- Experience developing and promoting learning resources, including through a learning management system (LMS).
- Experience managing grants and contracts, including compliance with state and federal guidelines.
- Knowledge of accessibility standards.
