Case Manager

Outreach is hiring a Full-time Case Manager at the Employment Center – Carol Stream to assess, coach, and serve the Employment Center participants, ensure case management services are successfully provided to all participants, take the lead on mental health consultation, and work with participants placed with Corporate Partners to ensure success.

Responsibilities:
- The Case Manager will carry a caseload of program participants who need support, instruction, and accountability to become successful employees.
- The Case Manager will conduct assessments to understand the basic needs, living situations, and any mental health concerns, as well as employment needs like work experience, job skills, and academic needs.
- The Case Manager will support students in establishing goals and becoming successful toward independence.
- Partner with worksite supervisors and transport students when needed.
- The Case Manager will serve as an instructor/facilitator for the Job Training Class.
- The Case Manager will maintain participants files, track progress, and provide reports as directed.
Qualifications:
- Master’s degree in Social Work, Counseling, or Human Services, or a related field.
- 3 to 5 years of direct service experience, including case management and work with youth or young adults.
- Demonstrated skill in assessment, engagement, and collaborative problem-solving with clients.
- Knowledge of public systems and resources.
- Strong organizational and communication skills.
- Sensitivity to the needs of individuals from low-income situations, high-risk families, and culturally diverse backgrounds.
- Basic proficiency in Microsoft Office.
- A valid driver's license and reliable transportation.
