Early Intervention Service Coordinator (EISC)

Part-time (29 hours/week)

$18/hour

 

GENERAL STATEMENT OF DUTIES: The Early Intervention Service Coordinator (EISC) is responsible for providing developmental screenings to children birth to three years of age and the coordination of early intervention services (EI) if the child is determined eligible. The SC will provide support to the family for the development and review of the Individualized Family Service Plan (IFSP) as well as planning for additional services that may be needed when the child transitions from HMG to family selected service options and/or school-age services. The SC is responsible for informing all families of their Parental Rights and Procedural Safeguards. In addition, the EISC must follow the Help Me Grow Program Rules as set forth by Chapter 3701-8 of the Ohio Revised Code. The Early Intervention program is supported by Department of Developmental Disabilities with federal funding from IDEA.

 

ESSENTIAL RESPONSIBILITIES:

1. Manages an active caseload of families and children participating in EI and spends approximately 50% of time providing direct service to families and children;

2. Establishes trusting relationship with families and children and all service provider agencies and organizations involved with families and children served;

3. Implements service coordination and supportive services per agency and program policies and procedures, including assessment of child and family needs, provision of information on procedural safeguards and parent’s legal rights, coordination of evaluation(s), development, implementation and evaluation of family centered IFSP, and facilitation of transition to family selected service options when child leaves the EI program;

4. Conducts developmental screenings (e.g., global development, nutrition, vision, hearing) and utilizes other family/child assessment tools per training;

5. Ensures that every infant or toddler referred to the EI program who is determined eligible for services has a completed IFSP within forty-five calendar days after initial program referral;

6. Obtains documentation of the qualifying diagnosed condition from the child's parent or health care;

7. Networks with area social services and agencies, understands eligibility requirements and makes appropriate referrals, based upon parental choice;

8. Prepares and submits accurate reports per EI program and F&CS agency requirements;

9. Maintains confidential case files and documents all service activities, contacts, goals, plans, referrals, and other relevant family information per EI program and F&CS agency requirements;

10. Secures reports and other pertinent information from relevant agencies and professionals involved with the family after obtaining proper consent forms signed by families;

11. Complies with agency policies and procedures, COA regulations, federal and state requirements and educational certification/registry requirements;

12. Attends all required DD trainings for service coordinators within one year of hire;

13. Maintains a SC credential without lapse every two years (i.e., 20 contact training hours);

14. Completes personal profile with verification on the Ohio Professional Registry;

15. Advocates for families /children;

16. Fulfills other duties, as assigned.

 

Requirements

 

QUALIFICATIONS:

1. Individual holds at least a Bachelor's degree from a council on higher education accredited college or university in a field related to working with pregnant women, infants or toddlers, or families or individuals with disabilities; 

2. Individual completes a personal profile with verification on the Ohio professional registry; 

3. Individual completes all required EI trainings on developmental screening tools, individualized family service plan training, RBI, and the principles of service coordination training as well as additional trainings as required by EI Supervisor;

4. Individual shall renew the EI credential every two calendar years from the date of initial or most recent credential renewal by completing at least twenty contact hours of training related to the role or target population of HMG, updating Ohio professional registry personal profile to include required training and obtaining verification by the Ohio professional registry;

5. Some evening hours and minimal required overnight travel;

6. Strong organizational, detail and interpersonal skills;

7. Excellent verbal and written communication skills;

8. Ability to work independently and be self-directed;

9. Valid Ohio Driver’s License and ability to meet agency requirements for driving insurability;

10. Must be able to meet agency requirements for criminal back ground check (Ohio Bureau of Criminal Identification & Investigation). 

 

MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS: Bachelor's degree from a council on higher education accredited college or university in one of the following fields of study or its equivalent:

  • Child and Family Studies/Child Development/Child Life/ Family Life Education;
  • Education inclusive of Early Childhood/Early Childhood Special Education/ Special Education/ Elementary Education;
  • Pre-kindergarten;
  • Deaf or Hearing Impaired/ Blind or Visually Impaired;
  • Hearing & Speech Sciences/Speech-Language Pathology;
  • Human Development/Human Ecology;
  • Human Social Services;
  • Nursing;
  • Occupational Therapy/Occupational Therapy Assistant;
  • Physical Therapy/Physical Therapy Assistant
  • Medicine/Physician Assistant;
  • Psychology;
  • Counseling;
  • Social Work.

MINIMUM EXPERIENCE REQUIREMENTS: Will consider candidates with at least two years experience working with families and children birth to three years of age either typically developing or special needs population.

 

PHYSICAL REQUIREMENTS:

  1. Must be able to lift 20 pounds;
  2. Must have valid driver's license with driving insurance.
