Pediatric Immunization Outreach Worker

This is a responsible position in the Health Department identifying, follow up and home visits to families in New Haven with children who are inadequately immunized.

The position will be responsible for promoting access to the Health Department’s services through community outreach, as well as parent organizing and group facilitation, distribution of education materials and resources on childhood immunization and disease prevention and community presentations and health fairs.

  • Assists local health care facilities with immunization tracking systems, assessments and data entry.
  • Conducts immunization assessments at local health care sites.
  • Other responsibilities will be to identify through outreach, families living in New Haven with children who are inadequately immunized or lack adequate access to health care because they do not have medical insurance.

Requirements include:

Graduation from an accredited college, preferably with a BS degree in Public Health; or pursuing a BS degree in Health and Human Services; three years’ experience working with health care providers and families with young children; or any equivalent combination of training and experience which provides the following knowledge, skills and abilities:

  • Comprehensive knowledge of health care community resources.
  • Knowledge of pediatric health care.
  • Maturity and experience in successfully working with infants and young children.
  • Knowledge of computers, specifically Microsoft Windows.
  • Ability to work with diverse socio-economic and socio-cultural groups.
