Physical Therapist
We are looking for a committed Physical Therapist to add to our Home Health Team. Our home health PT will perform OASIS SOCs, patient evaluations, reassessments and discharges as needed. The home health PT will collaborate with physical therapist assistants to carry out the prescribed Plan of Care.

Responsibilities
- Identify and meet patient goals and needs
- Develop care plans using a variety of evidenced based treatment techniques
- Provide quality, personalized and evidence-based care as well as proven interventions
- Motivate patients during treatment in order to help them function optimally
- Promote patient quality of life by improving ROM, strength, flexibility, balance, and coordination
- Consult, collaborate and practice with other health care professionals
- Evaluate effects, monitor and communicate progress
- Document patient care services

- Basic computer skills for our documentation software
- Valid license to practice physical therapy in the state of Ohio
Job Type: Full-time, Part-time
Salary: $45.00 - $65.00 per hour
