Care Management Specialist

SUMMARY OF DUTIES AND RESPONSIBILITIES

JOB FUNCTION: Under the direct supervision of the CMS Supervisor, Care Management Specialists (CMS) are responsible for all matters relating to the security of employees, residents, and visitors.  CMS’ are directly responsible for ensuring the continual accountability of all assigned residents, and a safe and secure environment that facilitates treatment and rehabilitation. CMS’ supervise resident drug and alcohol testing, visitation, recreation, laundry procedures, cleaning throughout the entire agency, transportation services, and other related duties as required. Facilitates security support for other treatment programs as required. CMS’ are expected to participate in other facility activities which foster a safe and secure environment that is conducive to the goals and mission of Community Assessment and Treatment Services.

Duties and Responsibilities:
  • Maintains operational logs, records, inventories, and files to facilitate security, safety, sanitation, and accountability;
  • Responsible for accurate, detailed, and timely reports and other documentation regarding resident and staff activity;
  • Participates in operational, programming, and other facility staff meetings as necessary;
  • Ensures that all minimum standards of the local/state/federal authorities are maintained within the scope of your responsibility;
  • Maintain an environment that is conducive to our agency philosophy and humane living conditions;
  • Enforces Residential Program Rules and Regulations in a firm, fair and consistent manner; and follows appropriate reporting procedures for violations;
  • Responds to resident needs and conflicts in a preventative fashion so as to reduce volatile situations;
  • Assures resident movement follows facility procedures; monitors residents entering and leaving program by following the established sign-in and sign-out procedures;
  • Responsible for handling crisis and emergency situations in program, calling for assistance when needed, and documenting all such incidents per agency policy;
  • Conducts visual head counts, facility shakedowns, and resident searches (including strip-searches) according to approved policies/procedures;
  • Monitors, identifies, and documents all individuals (i.e., visitors, outside agency personnel, officials, etc.) entering and leaving the Community Assessment and Treatment Services.
  • Supervises, directs, and evaluates the completion of resident housekeeping assignments within areas of responsibility. Ensures necessary cleaning supplies are available and utilized properly
  • Remains alert throughout work shift and maintains contact with personnel on duty to be aware of unusual situations that may arise and/or problems that need to be resolved;
  • Maintains close and continual coordination with the staff of all functional areas ensuring smooth facility operations, and that residents receive proper and timely services;
  • Ensures compliance with facility policies and procedures with regard to the use and storing of all sharp instruments, tools, toxic, corrosive, flammable substances, and other potentially dangerous supplies and equipment;
  • Properly notifies supervisor and maintains logs and reports of events, situations, and incidents which occur during the daily operations and programs of the facility;
  • Supervises, and documents weekly inspections in and around the facility to ensure the facility is clean, orderly, and in good repair;
  • Supervises resident during recreation, visitation, transportation, and other activities as required;
  • Follows procedure regarding the maintenance of transportation resources;
  • Supervises the maintenance of good order and discipline within the facility by following procedures and completing accurate timely documentation;
  • Responds in person, when required, to any problems and/or emergencies at the facility in a timely manner, regardless of hour; may have to work overtime;
  • Creates and maintains a working environment for effective communication among staff, including daily use of CATS’ email.
Minimum Skills and Knowledge Required:
  • Demonstrates knowledge of Substance Use Disorder (SUD) treatment & recovery, consumer engagement, healthcare, social services and criminal justice systems;
  • High School Graduate; Under Graduate education in Criminal Justice, Psychology or social work preferred, but not required;
  • Work experience that required the ability to implement effective operations, and the supervision of others which may have included military service, corrections, coaching, counseling, and the application of both positive and negative sanctions to facilitate behavior modification;
  • Experience with adult and/or juvenile offender/inmate/resident supervision and discipline, preferred;
  • Interpersonal communication skills requiring courtesy and tact when dealing with other staff, criminal justice agencies, local organizations, and the general public. Must be able to deal with residents and their families in a tactful, yet firm, fair, and consistent manner;
  • Ability to relate with people, listen, and then resolve problems effectively; ability to timely define problems, collect data, establish facts, and draw valid conclusions;
  • Experience with handling emergencies and potentially dangerous situations; ability to remain cool under pressure; ability to deal with aberrant behavior;
  • Ability to communicate effectively in writing; read, understand and write reports, memos, and policies;
  • Skills in effective written and oral communications; ability to orally give directions, speak on the telephone, read and write reports;
  • Ability to lift/carry up to 50 pounds as required;
  • Skill in the use of a personal computer and related office software preferred;
  • Valid Ohio Driver’s License Preferred.
  • Basic understanding of crisis interventions techniques
  • Ability to work independently, without supervision
  • Ability to use Microsoft word, email and agency electronic medical record system

Credentials / Academic Requirements:
 
·     High School Diploma or equivalent
·        At least one (1) year experience working cooperatively with others in a human service setting
·        Some training or experience in substance-related disorders preferred
·        CDCA preferred
·        Microsoft Word/Computer Proficient
