Case Coordinator
CASE COORDINATOR RESPONSIBILITIES:
Case Coordinator are assigned to each foster child and family in our program. The Case Coordinator is responsible for:
- The monitoring and protection of the children placed in care.
- They provide case management for individual children and home management of foster families.
- Case Coordinator provide on-call and emergency response supports, as well, to maintain the health and safety of children and foster parents in the home.
- They coordinate services for child(ren) as necessary to ensure all regulations are met.
- Case Coordinator attend monthly home visits in the foster homes, attend all team staffing’s, and create Individualized Plans to address each child’s treatment needs and how the foster home can support those needs.
- Case Coordinator are also trained in behavior programming and therefore, are responsible for creating the initial behavior protocol when a child enters into services.
- A primary responsibility of Case Coordinator is to provide monitoring and support to foster homes.
- Case Coordinators, also conduct home studies to assess the ability of foster homes to meet children's needs, all under the direct supervision of the Case Coordinator Supervisor.
- The Case Coordinator is also responsible for receiving all medical documentation and providing regulatory oversight to the children in care.
- This person can be the first contact for any medical-related questions and can serve as a resource for providers during regular business hours.
- The Case Coordinator is responsible for completing the initial case plan
QUALIFICATIONS:
- Bachelor's degree from an accredited college or university in human services or related field.
- Two years related experience working with children or two years’ experience in the human service field.
- Current driver's license, car registration and auto insurance of providing transportation to individuals receiving services.
- Current CPR/First Aid Certification as required by state.
- Must be at least 21.
- High integrity leadership skills.
- Communicate Effectively: Speak, listen and write in a clear, thorough and timely manner using appropriate and effective communication tools and techniques.
- Strong organizational abilities including planning, delegating, program development and task facilitation.
- Solve Problems: Assess problem situations to identify causes, gather and process relevant information, generate possible solutions, and make recommendations and/or resolve the problem.
- Behave Ethically: Understand ethical behavior and business practices, and ensure that own behavior and the behavior of others is consistent with these standards and aligns with the values of the organization
- Highly reliable/relatable.
- Possess the ability to handle high stress situations.
