Support Coordination Specialist
Job Scope
The Support Coordination Specialist protects services and supports from becoming institutionalized by promoting individual rights and responsibilities in conjunction with the system's needs that provide resources and support to them. They ensure that the person directs their services and supports as much as possible, even if the person has a legal representative. The Support Coordination Specialist ensures that the lives of the individuals we support are in alignment with their expressed needs and desires. They ensure continuity of support by being in a direct relationship with each individual and their legal representatives. They ensure that everyone’s team has the knowledge, training, and resources necessary to implement, maintain and document all support needs. Together with other Specialists, they promote a cohesive and respectful environment for each team that values our Direct Support Professionals and the work that they perform.
Work Schedule
• 40 hours per week
• Flexible Work Schedule based on team partnership assigned, generally working Monday through Friday.
• Non-Exempt, Overtime eligible position
• This position is a telework position, with primary work locations within the person receiving services’ home.
Wage
$23.45 per hour with annual wage increases based on longevity
