Creative Foundations is seeking Direct Support Professionals to provide direct care support to individuals with developmental disabilities.

Creative Foundations has been empowering individuals with developmental disabilities since 2001. Serving 200+ people across Ohio, we provide Homemaker Personal Care, Adult Day Services, and community support to help each person live their best life.

Must Meet Minimum Qualifications:
- Reliable automobile to transport individuals
- Satisfactory driving record from the Ohio BMV with no more than 5 points or citations over 9 miles an hour.
- Proof of High School Diploma or G.E.D.
- Must have a current Driver's License
- Proof of current automobile insurance listing applicant as a driver on the policy with state minimum liability amounts.
- Must be at least 21 years old.
- Satisfactory criminal background check.
